ICF-DD - Single story home for 16 individuals located in rural
Ashton, Il. Wheelchair accessible with 8 double bedrooms. Coed
with varied age group. Fenced in back yard. Focus of training
is on basic ADL skills. Functioning level is moderate to profound. |
1st Street Group Home:
ICF-DD - Single story home for 4 individuals in rural Ashton,
Il. Wheelchair accessible with 1 double and 2 single bedrooms.
Coed with varied age group. Focus of training is on basic ADL,
social, and behavioral development skills. Functioning level
is moderate to profound. |
Franklin
Grove, IL |
Franklin Grove Group Home:
ICF-DD - Single story home for 6 individuals located in small
rural community of Franklin Grove, Il. Currently all male residents.
Wheelchair accessible with 2 single and 2 double bedrooms. Varied
age group. Focus of training is on basic ADL and behavior development
skills. |

New Main Group
Home: ICF-DD - Single story home for 16 individuals located
in Dixon, Il. Wheelchair accessible with 8 double bedrooms,
shared bathrooms. Coed with varied ages. Services are geared
to individuals with extensive medical needs. Primary focus is
on basic ADL skills and physical rehabilitation. Home is considered
a secondary Day Service site when individuals are required to
stay home due to medical conditions or inclement weather. Additional
admission criteria: Must meet level 2 or 3 for medical services.

Tilton Park Group Home:
CILA - Single story home for 4 individuals located in Dixon,
Il. Home is not wheelchair accessible. Home has 1 double and
2 single bedrooms. Currently all female residents. Age group
is middle aged to elderly. Functioning level is mild to moderate.
Training focus is on social, community access, and independent
living skills. Additional admission criteria: Good mobility,
Female. |

Pine Acres Group Home: ICF-DD - Three story home
for 10 individuals located in Dixon, Il. Home is not wheelchair
accessible. Home has 5 double bedrooms with shared bathrooms.
Coed with varied age group. Training focus is on social, community
access, and independent living skills. Functioning level is
mild to severe. Additional admission criteria: Good mobility,
able to function without close supervision (due to 3 story construction).

8 individuals located in Dixon, Il. Home is not wheelchair accessible,
2 single and 3 double bedrooms. Coed with varied age group.
Functioning level is moderate to profound. Training focus is
on social, behavioral,
and ADL skills. Additional admission criteria: Good mobility.

Blackhawk Group Home: ICF-DD
- Single story home for 15 individuals located in Dixon, Il.
Home is not wheelchair accessible, 1 single and 7 double bedrooms.
Coed with varied age group. Training focus is on community access,
behavioral, social, and ADL skills. Functioning level is mild
to severe. |

Willow Lane: home for clients with autism. This four bed home has staff working there who have had specialized training from Southern Illinois University. These specialists from the university are considered to be at the forefront of the field in the area of autism. The opening of this home has met a critical need for clients who needed this kind of specialized living arrangement. The home is equipped with a sensory room with a variety of adaptive equipment that can be very helpful for clients who have elements of autism in their lives.

Pine Meadows: a six bed group home for behaviorally challenged and hearing impaired clients. Pine Meadows also has clients who have lived the majority of their lives in an institutional residential setting prior to their placement with Kreider. This home requires more staff direct care hours and increased nursing hours to meet the needs of the clients. As with our other homes, the opportunity to live in the community has opened many doors for the clients. |
Amboy,
IL |
Amboy Terrace Group Home: ICF-DD - Single
story home for 16 individuals located in small rural community
of Amboy, Il. Wheelchair accessible, 8 double bedrooms, coed
with varied age group. Services are geared for persons with
medical and/or geriatric needs. Home is considered a secondary
Day Service site when individuals are required to stay home
due to inclement weather or health conditions. Additional admission
criteria: Must meet level 2 or 3 for medical services. |

Boyd Group Home: ICF-DD - Single story home for
6 individuals located in rural community of Amboy, Il. Wheelchair
accessible with 2 single and 2 double bedrooms. Currently all
male residents of varied ages. Training focus is on teaching
appropriate social and behavioral skills. Functioning level
moderate to severe. |
Wasson Group Home: ICF-DD - Single story
home for 4 individuals located in small rural community of Amboy,
Il. Wheelchair accessible with 1 double and 2 single bedrooms,
coed with varied age group. Training focus is on appropriate
social and behavioral skills in addition to ADL skills. |

Division Group Home: ICF-DD - Single story home
for 6 individuals in rural community of Amboy, Il. Wheelchair
accessible, 2 single and 2 double bedrooms, coed with varied
age group. Training focus is on basic ADL skills.

Falls, IL |

Martin Road Group Home: ICF-DD - Single story
home for 6 individuals located in Rock Falls, Il. Home is for
persons with hearing and visual impairments. Handicap accessible
with 6 single bedrooms. Currently all male residents of varied
ages. Training focus is on basic ADL skills, mobility training,
communication skills, behavior development, and attention to
task skills. Functioning level is severe to profound. |
Swallow Court Group Home: ICF-DD - Single
story home for 6 individuals located in Rock Falls, Il. This
home is tailored for individuals with functioning levels of
severe and profound (male and female) with vision and/or hearing
impairments. Training focus is on basic ADL, communication,
mobility, behavior development, and attention to task skills.
Home has all modern accessibility and safety features. |
Avenue B.: This home is located in Rock Falls, along with two other 6 bed group homes that provide services to clients with developmental disabilities as well as having visual and hearing impairments. There are 6 clients living at Avenue B. and all six clients previously had lived in an institutional residential setting for most of their lives. Their lives now include attending the workshop on a daily basis, shopping in the community, visiting local restaurants, going to the recreation center to swim, visiting parks and even the local library. Staff use tactile signing (signing into the clients hands) to communicate with the clients. As with our other homes that provide services to clients with visual and hearing impairments, prior to opening this home, we obtained support and resources from the Helen Keller National Center located in New York.
